Westminster Or AKC National Championship Stipend Form

Criteria for $500 Westminster Or $250 A Stipend:

1. Applicant must be between ages of 9 and 19.
2. Applicant must submit a copy of their invitation/certificate with this form from the
   appropriate Limited Invitational Show Superintendent or the Official Organization.
   (ie: American Kennel Club, AKC National Championship, etc.)
3. Junior must provide a copy of their official school transcripts from the most current
   2 school semesters showing they have a cumulative GPA of at least 3.0 or
   equivalent.
4. Juniors must send a record of shows/dates listing the shows where they received
   their qualifying wins.
5. Applicant must apply on an official ACKCSC application form.
6. This is a one-time only award. Either the Westminster or AKC National Championship Stipend will
   be approved by the ACKCSC Board. Juniors may apply for both stipends, however
   only one will be awarded during the term of the Juniors’ eligibility.
7. All personal information listed below on this application will remain confidential.
8. Applicant should include information on additional activities with a Cavalier i.e.
   community service, regional club, therapy, rescue, education, etc. This information
   will be used in deliberation process but is not a requirement to qualify for the
   award.
